Ways to Submit Your Documents
- Fax: 515-987-2701
- Email: enrollment@waukeeschools.org (please send PDFs — no images)
- Visit: District Office at 560 SE University Ave., Waukee, IA 50263
There is also a black drop box in the parking lot at the District Office for use after hours. Once our staff receives and processes your documents, you will be emailed a link and code to complete the online enrollment process.
High School Students
If your student is entering grades 9-12, please schedule an appointment with the designated counselor at their school to set up your student’s class schedule. We recommend students bring their schedule and grades to that appointment. Any questions or concerns regarding academic courses, activities, athletics, clubs, etc. can be answered by calling the appropriate school.
We strongly encourage families to submit the health screening documents listed below to ensure our nursing staff can provide the best care for your child. You may submit the requested medical documents with other required paperwork to the District Office at the time of registration. If you don’t have the medical items completed at that time, don’t worry! You may bring any forms (or updated forms) with you during the Districtwide Open House Night when you meet your child’s teacher in late August. If that doesn’t apply to you, please submit health screen documents within 60 days of your school start date to your school nurse. Please include a physical (medical report) with blood lead test results, dental screening, and vision screening form. Please note, that documentation of a child’s recent immunizations or exemption is required for all new students. Review this handy student health requirements guide for all students in preschool through 12th grade so you’re prepared to meet requirements as your child grows.Required Enrollment Documents
